Who is required to have a physical exam?
Any boy or girl entering grades 6-12 who plans to participate in any athletic activity during the next school term. Physicals must be obtained by a student-athlete's family physician.

We have upgraded our forms based on the recommendations of the GISA and AAOP. All pages must be filled out and signed prior to the exam by both parent and student-athlete. The complete packet consists of two pages, both front and back and a clearance form (5 total). Parents must fill out the form completely and sign the consent areas and the physical form.
